Health Information Dental Requirements: All Kindergarten, 2nd and 6th graders are required to have a dental exam completed by May 15 of each school year.  If you are unable to complete the dental exam, you must complete a waiver.  The exam or waiver should be given to your school nurse by May 15. Vision Requirements: By October 15, any student enrolling in kindergarten or entering any other grade AND are coming from out-of-state, are required to have a vision exam completed.  If you are unable to complete the vision exam, you must complete a waiver.  The exam or waiver should be given to your school nurse by Oct. 15.
