Online Registration will be accessed from Skyward Family Access.  The first step is to make sure you have the ability to log into Skyward Family Access. If you have logged into Family Access before but no longer remember your account information please do the following:

• If you remember your email address which you used to create the Skyward Family Access account, click HERE

• Upon receiving that email you will next log in to your Skyward Family Access Account

• If you don't remember the login information or the email used to create your Skyward Family Access account, contact the appropriate school to reset your Skyward Family Access Account.

Early Childhood
Physical -Due by the 1st day of enrollment with up-to-date immunizations and lead screening

Kindergarten:
Physical.  (within 1 year of start of school) 30 days after registration
Official Birth Certificate (not hospital).  MUST be submitted at time of registration unless there is a special circumstance approved by an Administrator (new foster child, etc) 
Vision (exam or waiver)  
Dental (exam or waiver)  

2nd grade:
Dental (exam or waiver).  

6th grade:
Physical. (within 1 year of start of school) or 30 days after registration
Dental (exam or waiver).  

Transfer Students:
Current physical exam (must be performed by a physical within the state of Illinois) within 30 days of registration and vision exam.
