Health Requirements

School Physicals

In order to register, all Kindergarten students must have a completed school physical which shows up-to date immunizations. Sixth grade students are mandated to have their completed school physical, which shows up-to-date immunizations, turned into the Health Office by October 15, or your child will be excluded from school until the physical is turned in. Transfer students must have a current physical with up-to-date immunizations and performed by a physician within the state of Illinois within 30 days of registration.

Vision Requirements:

By October 15, any student enrolling in kindergarten or entering any other grade AND are coming from out-of-state, are required to have a vision exam completed.  If you are unable to complete the vision exam, you must complete a waiver. The exam or waiver should be given to your school nurse by October 15.

Dental Requirements:

All Kindergarten, 2nd and 6th graders are required to have a dental exam completed by May 15 of each school year. If you are unable to complete the dental exam, you must complete a waiver. The exam or waiver should be given to your school nurse by May 15.

Sports Physicals

All middle school students must have a completed Illinois sports physical on file in order to practice or participate in sports. Sports physicals are good for 395 days from the date of the exam. This physical must be on file at the school. Sixth graders may use their current (6th grade) school physical as their sports physical during their sixth grade year only.
