Community Health Care (CHC) will be offering onsite school physicals and immunizations for 5th graders in the spring. This will be the required physical and immunizations for 6th grade. If you are interested in having your child seen at the school, please fill out the online consent form, HERE. Students must have the consent form completed to be scheduled. Parents/Guardians are not required to be present if consent is received, but can participate in the visit via phone call.
